FAMILY SERVICES
Family Services provides comprehensive support to individuals and families facing medical and social challenges. We offer on-site resources and collaborate with a wide network of community-based organizations to ensure that every participant receives the personalized assistance they need to thrive.
Recognizing the increasing demand for support, our dedicated team works diligently to provide case management services tailored to each participant's needs. We assist with access to vital local and state resources, including healthcare, mental health, housing, food assistance, and more. Our case managers conduct thorough assessments to identify and address the basic needs of each family, guiding them through every step of the process.
To better serve our diverse community, we offer services in Spanish and Haitian Creole, ensuring language is never a barrier to receiving the help and care our participants deserve.
We are committed to empowering individuals and families to overcome obstacles and build a foundation for long-term success.
Case Management
We offer assistance and support services in the following areas:
- Referrals to: food pantries, medical and mental health services, domestic violence, and substance abuse.
- Tax Preparation and ITIN number
- ”Drive Only” license application
- Translation of birth certificates
- Advice on: tenant rights and housing laws
- Support with applications for SNAP, HUSKY, Section 8 and financial assistance
